I don't know about any of these. If they are, they probably aren't much known...

defrag has coop maps

Outpost is a single player mod, and you could play your maps in coop (as long as you had made your map with enough spawnponts).
I still have it on my computer, but I don't know if it's hosted anywhere anymore.

Q3Hunt had co-op, where you and other players either played a Left 4 Dead style mode where you ran away from enemies and tried to reach the end of the map. Come to think of it, it's basically left 4 dead without items or revives. It also had different enemies to fight and different enemy AI. The source code is also available, but it only really works if you compile to .qvm. DLL compilation will be glitchy, unless I'm missing something important.
