I always had this set to 0
How much of a benefit is it if I set it to, say 90 or 120?
Because right now when I do this, my view gets real grainy.
(Low detail settings)

You need to find the most appropriate r_displayrefresh for your resolution. Here are some reference points. (and turn off "wait for vertical sync")
800x600 -- r_displayrefresh 120
1024x768 -- r_displayrefresh 80 (I think)
1280x1024 -- 72, I think

Depends on your monitor, and your resolution.
You should set it as high as your monitor and resolution will allow.
If you can get it to 120, the game will seem very smooth.

Find out what your monitor supports for maximum. Try downloading your monitor driver (inf) so that windows loads it and knows the correct values for your monitor, then in display properties you can tick 'hide resolutions that your monitor can't display' and see which one is the maximum, then use that value.
Or you can use a refreshrate lock program like refreshforce:

(You won't need to bother with refreshrate game commands if you use this, except v-sync should still be off).

My monitors max refreshrate is 85
Resolution is 1280x1024
So I set r_displayrefresh to 85 right?

Yes.
Worth noting though that if you can do 85 at 1280, you can probably do 120 at 800x600. Most monitors can push higher refresh rates at lower resolutions.

try to sync ur maxfps with displayrefresh.
since 120 fps is good, you should go for 120 hz refresh.
if your monitor can't do this at high res, then lower the res.
i play at 640 because of this reason (also my computer won't run quake smoothly at higher res anyway)

But when you play osp all the time it don't matter, since it's locked at 125fps

Plus a small change like that (5fps) isn't noticeable in the 'best smoothness' category imo, least I wasn't able to tell the difference (when tested in baseq3).

I'd love to own a CRT that could push 240hz... But the market and supply for CRTs is pretty much gone. Very dissapointing.

Curious, what if you are on an LCD that is capable of doing 60Hz at 1600x1200. You wouldn't lower your com_maxfps to 60, would you. Would you do a multiple like 120 fps at 60Hz?

Running Q3 at 125FPS is for physics reasons mainly, you get the best movement at that framerate.
60 wouldn't produce the same motion. Q3's one of the faster FPS games and LCDs can really hold you back if you're trying to tweak all-out. Running at 120Hz on a CRT is a big difference though more taxing on the eyes, I find.

Quite possibly because older monitors don't do 120 @ that res, mine only does 100...

Talk about displayrefresh, i just change my monitor for a LCD ( ACL 19PO VIEWSONIC VX922 2MS ) and i had to change my displayrefresh for 75hz since its the highest i can get on 1024x768...
should i set it to 60hz or leave it at 75hz ?
