Are you using the 1.32 point release? Have you updated your video card driver to the latest version?

From Activision's support site:
Error on start - q_strncpyz: NULL src

Question
I get an error message, q_strncpyz: NULL src when I try to start.

Answer
# One thing known to cause this error is a missing or invalid registry entry for the OpenGL driver. Uninstall the video drivers and then reinstall the latest drivers for your video card.

NOTE: Activision does not provide support for the uninstallation or reinstallation of any driver type. It is recommended that you contact your hardware manufacturer for more assistance.

# This can also occur if you have dual monitors, disable the other monitor while playing Quake III. To do this:
1) Right-click on the Windows desktop, and left-click on Properties.
2) Click the Settings tab.
3) Uncheck the box that indicates you want the desktop extended to the other monitor.
4) Click apply and finish the operation.

# If you have a VIA chipset motherboard then go to http://www.viatech.com and get the 4 in 1 drivers for your motherboard. If you need help installing these motherboard drivers then contact your motherboard manufacturer.

# Perform an uninstall and reinstall with the latest patch.
1) Uninstall the game.
2) Delete the game folder.
3) Reinstall the game
4) Download and install the latest patch from http://www.quake3world.com.
