Hiya everyone, its been a while.

My sound is corrupted when I start Q3 and ingame as well.

The pounding id logo intro clips, skips or is altogether silent and everything else is clipped and high pitched.

Other games (including RTCW) work just fine.

I tried renaming my config so that Q3 would create a new one, but still no joy.

Do you think my pak0 is corrupted?

Go to your utilities folder and open "Audio/MIDI setup". Then make sure the sound output port that you are using (most likely the built-in default one) is set to 44100Hz (or 44.1KHz) and you should be golden.
