I always had this set to 0
How much of a benefit is it if I set it to, say 90 or 120?
Because right now when I do this, my view gets real grainy.

You need to find the most appropriate r_displayrefresh for your resolution. Here are some reference points. (and turn off "wait for vertical sync")
800x600 -- r_displayrefresh 120
1024x768 -- r_displayrefresh 80 (I think)
1280x1024 -- 72, I think

Find out what your monitor supports for maximum. Try downloading your monitor driver (inf) so that windows loads it and knows the correct values for your monitor, then in display properties you can tick 'hide resolutions that your monitor can't display' and see which one is the maximum, then use that value.
Or you can use a refreshrate lock program like refreshforce: http://www.pagehosting.co.uk/rf/
(You won't need to bother with refreshrate game commands if you use this, except v-sync should still be off).

Curious, what if you are on an LCD that is capable of doing 60Hz at 1600x1200. You wouldn't lower your com_maxfps to 60, would you. Would you do a multiple like 120 fps at 60Hz?

Running Q3 at 125FPS is for physics reasons mainly, you get the best movement at that framerate.
60 wouldn't produce the same motion. Q3's one of the faster FPS games and LCDs can really hold you back if you're trying to tweak all-out. Running at 120Hz on a CRT is a big difference though more taxing on the eyes, I find.

Talk about displayrefresh, i just change my monitor for a LCD ( ACL 19PO VIEWSONIC VX922 2MS ) and i had to change my displayrefresh for 75hz since its the highest i can get on 1024x768...
should i set it to 60hz or leave it at 75hz ?
