r_stereo is not a setting that is related to sound. All variables that are prefixed with r_ are related to graphical settings (the r probably stands for rendering). I'm not sure what the r_stereo setting does, but it may be related to some (obsolete) stereoscopic rendering function.
All sound related settings are prefixed with s_

For the interested:
/r_stereoEnabled 1
/r_stereoSeparation 64
Don your 3D glasses and enjoy the stereoscopic goodness (and headache afterwards).
Requires ioquake3. But you should have that installed anyway.

I have added the following to an AUTOEXEC.CFG
seta r_stereoEnabled 1
seta r_stereoSeparation 64
However, there is no difference in Quake 3.
What is ioquake3... i dont think i have it.

BTW and for great justice: /r_anaglyphmode 1

OK, got ioquake 3.
Installed, updated and copied the pak0 file.
I created an autoexec.cfg file containing:
seta r_stereoEnabled 1
seta r_stereoSeparation 64
seta r_anaglyphmode 1
Still no luck.
Is there a guide out there?

You might need to tweak r_stereoseparation (less is more IIRC).

Please don't forget: ioquake has its config files not in baseq3!
